ACTIVITY AID - FULL TIME

A Long Term Care Facility dedicated to providing residents with quality health care, maintaining their well being, strength and comfort. The services provided include skilled nursing, rehabilitation, restorative services, social services, planned activities, dietary and respiratory services. While striving to meet and exceed the standards of residents and families we also aim to provide a satisfying workplace.

JOB DESCRIPTION:
 Participate with all members of the department when large group activities are planned.
 Transport resident to activities on the nursing unit or other rooms in the facility.
 Provide a list for nursing staff of residents participating in special activities.
 Document on the Activity Form the resident’s response to activities they participate in.
 Accompany residents on community, out of facility activities as assigned.
 Complete the assessment of resident’s need for various activities for the MDS and Care Plan.
 Coordinate plan of activities with the Director of Activities.
 Plan of activities includes recreational, diversional, religious, intellectual, functional, and service.
REQUIREMENTS:
High School Diploma.
Experience Preferred.
Must work well with people.
