This role is responsible for overseeing daily clinical operions, supporting and mentoring field staff, and ensuring high-quality pient outcomes and regulory compliance. OFFERING NEW Salary Res: $85,000 - $115,000 (Plus Annual Bonus)

Qualificions:

Licensed RN with current licensure to practice professionally within the ste.
Gradue of an accredited school. BSN degree is preferred.
Three years management or supervisory experience in health care setting, preferably home health care.
Complies and maintains current CPR certificion.
Valid driver’s license and proof of insurance is required

Wh You’ll Do:

Receives case referrals. Reviews available pient informion reled to case, including disciplines required, to determine home care needs. Assigns approprie clinicians to case.
Reviews and evalues each case by reviewing the services provided by clinicians, conferences, record review, discusses and verifies impressions, instructs and guides clinicians to promote more effective performance and delivery of quality home care services, and is available all times during opering hours to assist clinicians as approprie.
Reviews pient’s clinical diagnosis, medicions, procedures and clinical course.
Assists clinicians in establishing immedie and long-term therapeutic goals, in setting priorities, and in developing plan of care.
Conducts or assists with quarterly record reviews and communices findings and recommendions to Executive Director and approprie organizion personnel.
Assists in the formulion, revision, implemention, and evaluion of policies and procedures, as well as stregic goals and objectives.
