Be the Best Hospice After Hours Admission RN You Can Be

If you meet these qualificions, we would love to meet you:

• Current licensure as a Registered Nurse in the applicable ste is required, with an associe degree preferred. CPR certificion is recommended (required for direct pient care in Georgia), along with prior experience in hospice, home health, medical-surgical, or populion-specific nursing such as gerirics or pedirics.
• Must demonstre a willingness to participe in an on-call rotion and maintain reliable transportion, valid driver’s license, and ste-required liability auto insurance.
• Strong communicion and organizional skills are essential, along with proficiency in Microsoft Office Suite and EMR systems. The ability to collabore effectively within an interdisciplinary team is required.
• This role involves regular physical activity, including lifting pients over 200 lbs with assistance, transporting supplies and equipment up to 20 lbs for distances of 500 feet, and frequent standing, walking, sitting, stooping, and reaching.

Responsibilities:

• Conducts comprehensive nursing assessments and collabores with pients, families, physicians, and the Interdisciplinary Group (IDG) to develop, implement, and evalue individualized plans of care in alignment with regulory requirements and the Case Management Recipe.
• Maintains timely and accure documention, ensures care plans reflect current pient stus, and communices effectively with the care team to coordine optimal use of resources, especially in complex or high-risk situions.
• Provides educion and emotional support to pients and caregivers based on individual needs, and actively participes in on-call rotions, admissions, and care transitions as directed.
• If serving as a Care Manager, coordines all aspects of pient care including scheduling, medicion management, plan of care oversight, IDG collaborion, and supervision of LPNs, LVNs, and Hospice Aides; participes in IDG meetings and pient/family conferences.
• If serving in an On-Call capacity, responds to triage calls, performs visits and assessments, coordines care with the Administror On Call, and may tend IDG meetings and family conferences; also participes in training, committees, and other duties as assigned within licensure scope.
